Reporting Form for Title IX (Sexual Harassment), Harassment, Discrimination, and Retaliation

REPORTING TO THE UNIVERSITY

If this is an emergency or an incident that requires immediate attention, please call Campus Security and Public Safety at (509) 313-2222 or dial 911.

All reports are encouraged in order to stop the behavior, prevent its re-occurrence and retaliation, address the behavior through our student and employee disciplinary systems, and to remedy the effects.

Please use to the form below to report allegations of behavior that violates Gonzaga's Title IX Sexual Harassment Policy or our Harassment and Discrimination Policy, which includes gender-based harassment and discrimination that does not fall under Title IX as well as all other forms of harassment and discrimination. Once a report is received, it will be reviewed by the Title IX Coordinator or Authorized University Official. The complainant, or agrieved party, will be contacted by our office via their official university email within two business days.

For every report to the University, the Title IX Coordinator will reach out to the person(s) affected to explore options for support, discuss the formal complaint and grievance process, and the options for alternative resolutions. Filling out this form creates an incident report and does not constitute a formal complaint.

Examples of behaviors that constitute a violation of University Policy include, but are not limited to, sexual assault, sexual harassment, unwanted sex-based communication, stalking, dating and domestic violence, sexual exploitation, quid pro quo, and sex or gender discrimination based on a person's sex, gender identity or sexual orientation.

Examples of behaviors that constitute a violation of the University Harassment and Discrimination Policy include, but are not limited to: physical or verbal conduct, name-calling, slurs, comments, jokes, or other conduct which may be physically or emotionally threatening, harmful, or humiliating based on one's protected characteristics or identity. Protected characteristics include: race, ethnicity, color, religion, national origin, sex, gender, gender identity, marital status, sexual orientation, age, disability, and military status.
